Acrylic Spas Manufacturing Defect: Structural crack. Surface issues like; blisters, map cracking, egg shell cracking.

- Date of installation
- Date you noticed the issue
- Image of receipt or proof of purchase
- Images of your pool & spa including: Bird’s-Eye View of pool & spa, Front View of spa, Structural Crack detail images
- Image of Warranty (if available)
- Your contact information, including Email, Phone Number and Home Address
- Dealer / Installer contact information, including Email, Phone Number and Home Address
